Waiver, Release of Liability & Media Consent
 
Applicable to All Participants and All Future Victoria Soccer Project Activities
 
 
Important Notice
 
By registering for any Victoria Soccer Project activity, you acknowledge that you have read, understood, and agreed to the terms of this Waiver, Release of Liability, and Media Consent.
 
This waiver applies to the current registration and to all future events, sessions, trainings, practices, games, camps, clinics, evaluations, and activities organized or hosted by Victoria Soccer Project, unless revoked in writing.
 
 
Activity Description
 
Victoria Soccer Project activities include, but are not limited to: youth and adult soccer training, practices, games, camps, clinics, tryouts, evaluations, community events, and any related activities operated by or in partnership with LA Monsters Sports Academy, Inc., at any location.
 
 
Assumption of Risk (All Participants)
 
Participation in athletic activities involves inherent risks, including but not limited to physical contact with other participants, falls, collisions, overexertion, weather conditions, use of athletic facilities and equipment, and other risks associated with sports activities.
 
You acknowledge that serious injury, permanent disability, or death may occur, and you voluntarily and knowingly assume all risks, whether known or unknown, arising from participation.
 
 
Release and Waiver of Liability (All Participants)
 
To the fullest extent permitted by California law, you agree to waive, release, and discharge LA Monsters Sports Academy, Inc., Victoria Soccer Project, and their officers, directors, employees, contractors, coaches, volunteers, agents, sponsors, affiliates, and facility providers from any and all claims, demands, liabilities, or causes of action arising out of or related to participation in Victoria Soccer Project activities.
 
This release includes claims arising from negligence, and applies to personal injury, illness, death, or property damage.
 
This waiver is intended to be continuing in nature and applies to all current and future Victoria Soccer Project activities.
 
 
Medical Disclaimer and Authorization
 
Victoria Soccer Project does not provide medical insurance for participants.
 
In the event of an emergency, reasonable efforts may be made to contact a participant or parent/guardian. You authorize Victoria Soccer Project to seek emergency medical treatment if necessary and agree to be financially responsible for any medical care provided.
 
 
Photo, Video & Media Consent
 
By registering, you grant permission to LA Monsters Sports Academy, Inc. and Victoria Soccer Project to photograph, video record, and otherwise capture images or likenesses of you and/or your child during activities.
 
You authorize the use, reproduction, and publication of such media for marketing, promotional, educational, website, social media, fundraising, and media purposes, without compensation and without time or geographic limitation.
 
 
Adult Participants (18 Years of Age or Older)
 
If you are 18 years of age or older, by registering you confirm that:
 

• You have read and understood this waiver

• You voluntarily assume all risks

• You agree to all terms and conditions set forth above

 
Your registration constitutes your full and binding acceptance of this waiver.
 
 
Minor Participants (Under 18 Years of Age)
 
If the participant is under 18 years of age, registration must be completed by a parent or legal guardian.
 
By registering a minor, the parent or legal guardian confirms that:
 

• They have read and understood this waiver

• They consent to the minor’s participation

• They agree to all terms on behalf of the minor and themselves

 
This waiver binds the minor, the parent or guardian, and their respective heirs and representatives.
 
 
Code of Conduct
 
Participation in Victoria Soccer Project activities is conditioned upon respectful and safe behavior by participants and families. Victoria Soccer Project reserves the right to remove any participant whose conduct is unsafe or contrary to program values.
 
 
Digital Agreement
 
By completing registration, you acknowledge that your electronic acceptance of this waiver is legally binding and has the same effect as a handwritten signature.
